APPLICATION AND FORMS
What paperwork do you need? Click on whichever of the following describes your situation to see a list of exactly what you should fill out and how to get it:
New Work-Study Tutor
Returning Work-Study Tutor
New Volunteer Tutor
Returning Volunteer Tutor
You may not begin tutoring and will not be paid until all of these forms are turned it. If application and authorization form are turned in after the New Tutors Mandatory Meeting, your forms will not be processed until you have met with the DC Reads Coordinator.
- DC Reads Application--Be sure to answer questions for new tutors and attach.
- Federal Work-Study Authorization Form--Pick up at the Campus Employment Fair on September 2nd in the Great Room in the Pryz between 10 and 2.
- I-9 and Appropriate Forms of ID--Can be turned in at the Campus Employment Fair, DC Reads Mandatory Meeting, or Office of Career Services.
- TB Test Results--Must be taken from the past year. TB Testing will be available immediately before the mandatory meeting.
